Transition Matters

Our family conference took place on Saturday 2nd April 2011 at the Toorak Hotel in Torquay. This was the last conference to take place down in the South of England as future conferences will be held in Birmingham.

ASUK are currently looking to improve transition services for our young people, ensuring that they are at the centre of all decisions made about their future.

We want to incorporate all aspects of a person’s life including health, education, social activities, friends, family, support services and future career options.

The move from Paediatric to Adult Services is often a very daunting experience and our aim is to start this process from a young age to enable our young people and their families time to explore new ideas and develop new relationships.

The conference identified some of the services available to support people through transition and hopefully answered many of your questions or addressed any worries families had about the future.
